In Civilization 4 how can I do better in wars?

I need some help with Civ 4. I normally do great until about the medieval era, as soon as i get attacked once or in one little conflict i crumble. I don't have any expansion packs just regular civ. 4. I'd also like some tips on how to build a good army and attacking other people, thanks
